About Us

The American Institutes for Research (AIR) developed this Toolkit and Triple Aim visualizations based on dozens of interviews with quality improvement and measurement experts across the United States and field work with several community health centers in California.

Amy Boutwell, M.D., M.P.P., founder of Collaborative Healthcare Strategies, articulated the steps to take when measuring the Triple Aim. Dr. Boutwell offers guidance and advice based on her extensive experience supporting health stakeholders across the country, including the New York State Delivery System Reform Incentive Payment Program, the Centers for Medicare & Medicaid Services, the Agency for Healthcare Research and Quality, the Massachusetts Health Policy Commission, the Maryland Hospital Association, and others.

We are indebted to our partners at Community Health Center Network, Open Door Community Health Center, and West County Health Center for working with us and sharing their data so we could understand how to visualize health, experience, and cost simultaneously. We also thank them for advising us and providing input and feedback on this Toolkit.

We are also grateful to Asian Health Services, Neighborhood Healthcare, North County Health Services, and Northeast Valley Health Corporation for sharing their experiences measuring and demonstrating value at their community health centers for inclusion in this Toolkit.

We thank the following organizations for sharing information about their programs and providing input on the Toolkit:

The Toolkit was made possible by funding from Blue Shield of California Foundation.
